SECTION 01

10-Year Puppy Replacement Program

To qualify for our Puppy Replacement Program, you must obtain and maintain major medical pet insurance for your new puppy for a minimum of six (6) months from the date of purchase.

PROGRAM REQUIREMENTS

  • Insurance Enrollment: You must enroll and activate coverage prior to your puppy's arrival.
  • Proof of Coverage: You must provide documentation confirming that major medical pet insurance was in effect for the first 6 months of ownership.

PROGRAM BENEFITS

With valid proof of insurance as described above, you will be eligible to receive a replacement puppy for up to 10 years from the original puppy's birth date in the event of any Hereditary or Congenital abnormalities when diagnosed and documented by a licensed veterinarian impacting the puppy's ability to live a normal life.

REPLACEMENT DETAILS

  • The replacement will be provided in the form of a store credit equal to the original invoice total.
  • This store credit can be used at any time and does not expire.
  • No cash refunds will be issued under this program.

SECTION 02

Cash Medical Reimbursement Program

As a thank you for being a responsible pet owner, Lone Star Pups offers cash medical reimbursement for conditions related to hereditary or congenital disorders as well as sickness, subject to the following terms.

ELIGIBILITY REQUIREMENTS

  • The condition must bediagnosed and documentedby a licensed veterinarian.
  • You mustmaintain active major medical pet insurance coveragethroughout the reimbursement period, which is up to 10 years from the original puppy's birth date.
  • Your puppy must remainunder the regular and continued care of a licensed veterinarian.

The following conditions are limited to 3 years from date of birth for cash medical reimbursement: Cancer, Pancreatitis, Auto Immune, Diabetes, IBD (intestinal disease), Seizures.

COVERAGE DETAILS

  • Deductible Reimbursement: We will reimburse 100% of your insurance deductible (up to $300).
  • Co-Pay Reimbursement: We will reimburse 100% of your co-pay (up to 20% co-pay).
  • Lifetime Cash Medical Reimbursement Limit: Reimbursements are capped at the original reservation price (excluding shipping and any additional services or products).

IMPORTANT NOTES

  • No vet clinic restrictions apply - you may use any licensed veterinarian.
  • The claim must be approved and paid by your major medical insurance provider before reimbursement from Lone Star Pups can be issued. All claims denied from the insurance provider will be denied by Lone Star Pups.
  • For non-emergency issues,prior authorization is required from Premier Pupsto qualify for reimbursement of deductible and co-pay.
  • To request approval, email the diagnosis and recommended corrective procedures to: [email protected]
  • Once approved, provide Lone Star Pups your EOB (explanation of benefits) from your insurance provider, and Lone Star Pups will issue cash reimbursement for co-pay and deductible within 15 days.
SECTION 03

Additional Coverage for Sickness / Illness

  • The puppy is guaranteed to be free from illness and Lone Star Pups provides a replacement puppy per the terms of the replacement program for any sickness-related death in the first 10 days.
  • Cash medical sickness reimbursement is made available for the first 10 days after the puppy's arrival, providing the conditions below are met.

WHAT YOU MUST DO

If your veterinarian detects any illness within 10 days, you must notify Lone Star Pups within 24 hours. Contact: 979-314-9974 7am-11pm EST

INSURANCE COVERAGE REQUIREMENT FOR CASH MEDICAL SICKNESS COVERAGE

  • Lone Star Pups will cover 100% of your co-pay and deductible for any illness within the first 10 days if you have enrolled in major medical pet insurance (with no wait period provisions) before your puppy arrives.
  • Remember: immediate action and proper insurance enrollment are critical to receive cash medical sickness coverage.
SECTION 04

What You Must Do

In addition to enrolling in major medical pet insurance, you are required to complete the following steps:

  • Vet Visit Within 7 Days. You are required to have your puppy examined by a licensed veterinarian within 7 days of receiving the puppy. This is crucial to uphold the replacement puppy and cash medical reimbursement program.
  • No Vaccines at First Visit. Do NOT allow vaccines to be administered at the first vet visit. Vaccines should be delayed until the second visit, as your puppy may already be under stress from travel and a new environment.
  • Bring All Provided Records. Your vet will review the health records Lone Star Pups gave you. Based on the exam and records, the vet will set up a custom preventative care schedule (e.g., vaccinations, deworming, etc.).
  • Don't Delay. Lone Star Pups stresses that you should not postpone this initial vet visit.

WHY THIS MATTERS

  • It protects your puppy's health and ensures a smooth transition.
  • It helps validate any claims under the 10-day health guarantee.
  • It supports a proper care and vaccination timeline without overloading the puppy too early.

Some health concerns may not become apparent until later in your puppy's development. This can happen with any puppy — regardless of breed, sex, or genetic background. These issues are often the most difficult to understand and accept. We want you to know that we are here to help. SECTION 05

Important Information Regarding Coverage

HIP DYSPLASIA

Costs arising from treatment for hip dysplasia are not covered by major medical pet insurance if clinical signs appear within the first six (6) months of coverage. However, if your puppy is insured and diagnosed with hip dysplasia during this period, you may qualify for cash reimbursement up to the value of the puppy if corrective surgery is performed.

HYPOGLYCEMIA / ANOREXIA

Claims arising from hypoglycemia are not covered under this program.

IN THE CASE OF SUDDEN DEATH

A necropsy is required in order to file a full replacement claim. As a courtesy, Lone Star Pups will offer a 50% discount (up to $2,000) on the purchase of a new puppy if you are unable or choose not to have a necropsy performed.

FETCH PET INSURANCE

Fetch Pet Insurance also provides a $1,000 cash reimbursement in the event of sudden death, provided you have active coverage.

BREEDING STOCK

If you choose to breed your puppy, that is at your own discretion. We do not guarantee the puppy to be fit for breeding. We reserve puppies with the intention that they will be family pets.

IMPORTANT: You must enroll in major medical insurance to qualify for cash medical reimbursement and puppy replacement.

Dispute Resolution

Customer agrees that the exclusive means of resolving any dispute, controversy, or claim arising out of, or relating to, your purchase of a dog from Lone Star Pups is either binding arbitration administered by the American Arbitration Association or, if applicable requirements are met, small claims court.
